Your medical records are the property of the UND Center for Family Medicine. Information in your medical chart is confidential and the content of your medical record is released only with your hand-written consent. No information is released without your signed authorization.

Patient Portal Information

The Patient Portal is a secure, web-based tool that provides you with 24/7 internet access to your electronic health record and lets you communicate with your provider about appointments and changes in your health status. Here at the UND Center for Family Medicine, we create and store your electronic record within the Cerner EHR (electronic health record) through Trinity Hospital. If you already have access to your electronic health record through Trinity, you already have access to your records here at the UND Center for Family Medicine and you do NOT need to register again.
